Household Membership Application

This membership covers up to 5 adults living in the same residence.  Please note that children under the age of 18 do not need to apply for membership.  Renewals for this option are $90 no matter how many are registered.  Membership is for the calendar year Jan 1 through Dec 31.  Applications received during the last quarter of the year (Oct, Nov, Dec) will be grandfathered for the following year.

Instructions for new membership:

1. Complete these questions below. 
2. Click "Add to Cart"
3. Go to Cart and checkout (pay for your membership)
4. Look for an email from EOC, in this email you can download your New Membership Liability Release. (see image below)  Please print, sign and email/submit to EOC.
5. At this time EOC officers will have received your membership application and will contact you to schedule a time to meet, review your application and complete the membership process.  You will also be asked the the names, dates of birth, and contact information for all applicants covered.
